Referral to Hospice Services
If you or a member of your family has a life limiting illness and need the services of St. Helena Hospice please complete the referral form available by clicking here and return it by fax or post to the number on the form. Alternatively you can contact the Referral Secretary on 01206 845566 between the hours of 8am – 4pm and the relevant details will be taken and the request processed at the next referral meeting.
