Day and Community Services

St Helena Hospice is able to provide flexible and dynamic day and community services from two Community Team sites: Colchester and Clacton.

Both Centres are the bases for teams offering specialist information, support and advice to people diagnosed with a life-threatening illness.

This support is also offered to family, friends and carers of the patient. The hospice teams work closely with General Practitioners, District Nurses, Social Services, Hospital staff and Nursing Homes. The team includes specialist palliative care nurses, social workers, counsellors, doctors, occupational therapist and physiotherapist.


Our specialist help may be of a nursing/medical nature, about practical day-to-day living issues, or may be more psychological in its emphasis.

Macnurse home visits can be in different sorts of homes.


Most people who visit our Day Service Centre gain immense support from spending time with others who are living in a similar situation. Many people attend on a regular day each week, others on a more flexible basis. There is a day at each Centre tailored specifically for younger people - from teens up to forty-something - and those who have young families.

From the time of diagnosis, and throughout a person's treatment, we aim to enable a patient and their family to confidently face the challenge of living with a life threatening illness and retaining/optimising their quality of life.

   

After an initial assessment, the team will work with a patient and their family to agree a plan that will form the basis of our support assistance. This is reviewed on a regular basis, usually 8 weekly.
